About this job:
Community Health Workers (CHWs) build relationships with patients and help them with things that affect their health, like food, housing, or transportation. You’ll meet with people one-on-one, talk with them about their needs, and help connect them to community programs or support.
You don’t need a medical degree, but you do need to care about others and have real-life experience finding help or using services like food stamps, transportation, housing support, or working through tough life challenges.
What You’ll Do:
- Talk to patients and build trust. Learn their story. Get to know them as people and help them do the things that they think will improve their health such as offering a listening ear, connecting them to resources like food delivery, or working out with them at the YMCA
- Help them set goals that are important to them.
- Go with patients to appointments or meet them in their home or neighborhood.
- Help patients with problems like finding food, paying bills, or getting housing.
- Make phone calls, fill out forms, or help with paperwork if needed.
- Work with other Healing Hands staff to support the patient.
- Keep simple notes in a computer about who you helped and how.
- Join weekly team meetings and training.
What You Need:
- High school diploma or GED
- Life experience that helps you understand what our patients are going through
- Good at talking and listening
- Able to manage your time and stay organized
- Willing to learn new things
- A car and driver’s license
- Community Health Worker certification not needed to apply but a plus
- If not certified, must obtain certification within 1 year of employment
Schedule: Monday through Friday, 8:00 am–5:00 pm
Pay and Benefits:
- $16–$18/hour if you do not yet have a CHW certification
- $19–$22/hour if you are already certified (pay depends on experience)
